Client Referral Policy
Terms and Conditions

Purpose

The referral policy aims to incentivize external partners and independent contractors to refer potential clients (healthcare facilities & anesthesia management companies) to KREWE Anesthesia, LLC (“KREWE”). This policy outlines the terms and conditions under which referral bonuses will be awarded for successful client referrals.

Eligibility

  1. Who Can Refer: This program is open to external partners and independent contractors. W2 employees of KREWE and its affiliates and subsidiaries are not eligible for referral bonuses.
  2. Who Can Be Referred: Any healthcare facility or management company with anesthesia staffing needs that is not already a client of KREWE or has not been contracted with KREWE for services within the past 12 months.

Referral Bonus Structure

Client Referral Bonus: Up to $1,000 for each new candidate placement at a referred client that completes a minimum contract period as specified by KREWE, up to $15,000 per client referral.

Referral Process

  1. Submission: Referrals must be submitted via the official referral form on the KREWE website. Required information includes the name, contact details, and relevant information about the referred client.
  2. Confirmation: Upon submission, referrers will receive a confirmation email with a tracking number. The referred client must confirm the referral from the referring party.
  3. Follow-Up: Our sales team will contact the referred client within five business days.
  4. Tracking: Referrers can track the status of their referrals through our online dashboard, which provides real-time updates on the referral process.

 

*Multiple Referrals: If a client is referred by more than one person, the client must determine in writing the sole referring party to whom the bonus is paid.

Payment Terms

  1. Conditions for Payment: The referral bonus will be paid out after the referred client signs a contract with KREWE, engages in staffing services, and a new placement candidate completes 500 hours on assignment within one year of the date of referral.  The bonus is contingent upon the client’s successful engagement without any contract breaches.
  2. Payment Schedule: Referral bonuses will be processed and paid out in the payment cycle following the completion of the engagement period.
  3. Required Documentation: The referring party must provide a valid W-9 and current contact information within 30 days of the client’s contract initiation.
  4. Taxes: Referral bonuses are subject to applicable taxes and will be reported as income on the applicable IRS form (1099-MISC).
  5. Payment Method: Bonuses may be paid via EFT or check, at the company’s discretion.

General Conditions

  1. Program Changes: KREWE reserves the right to modify or terminate the referral program at any time without prior notice. Changes will be communicated via the company’s official communication channels.
  2. Confidentiality: Referrers must maintain the confidentiality of sensitive information and not disclose their referral status publicly.
  3. Non-Eligible Clients: Clients who have been referred previously or are already in the company’s client engagement process are not eligible for referral bonuses.
  4. No Stipulations: When providing a referral, no stipulations as to the type of services or specific agreements the client must engage in are allowed.
  5. Unlimited Referrals: There is no limit to the number of client referrals a referring party can make or bonuses a referring party can earn (other than the limit of $15,000 per client referral), subject to meeting all eligibility requirements.

Disqualifications

  1. Conflict of Interest: Referrals from individuals who have a direct or indirect influence on the client engagement decision will not be eligible for the referral bonus.
  2. Incomplete Submissions: Incomplete or inaccurate referral submissions may result in disqualification from the program.
  3. Fraudulent Activity: Any attempt to manipulate or abuse the referral program will result in immediate disqualification and potential legal action.
  4. Non-Compliance: Referrals that do not comply with company policies and guidelines will be disqualified from receiving bonuses.

Liability

To the fullest extent permitted by law, neither KREWE nor its affiliates will be liable for damages arising out of or in connection with this offer. This includes compensatory, direct, indirect, or consequential damages; loss of data, income, or profit; loss of or damage to property; and claims of third parties, regarding which the referral source agrees to hold KREWE harmless.

Legal and Compliance

  1. Certification: By referring a client to KREWE, the referring party certifies that they are not currently a government employee or active military, and that the referral and receipt of any bonus for such referral do not violate any applicable employment obligations to which the referring party is subject.
  2. Compliance with Laws: The referral program is subject to all applicable federal, state, and local laws and regulations. Referrers are responsible for ensuring their participation complies with any laws applicable to them.  To the extent that any referral payment violates any state or federal laws, the referral source agrees to refund KREWE any referral fees in violation of those laws.
